Once your appointment is scheduled, we’ll send your new patient forms directly to you for completion before your visit. These forms help us prepare for your appointment and ensure we’re providing the most accurate and effective care.

Insurance Notice

We are currently in the process of transitioning out of network with all dental insurance providers. While we’re still happy to file claims on your behalf, any reimbursement will be sent directly to you. We believe in transparency and will gladly provide a treatment plan and cost estimate before beginning any care.

To support patients without insurance or those looking for more predictable costs, our office will soon be introducing an in-house savings plan designed to make quality dental care more accessible and affordable. Stay tuned for details, or ask our team during your visit!
